Job Description

Organizational Summary:


USF Division
of Child Development resides within the Department of Pediatrics in the College of medicine. The Division Chief reports to the Department
Chairman of Pediatrics. The Division of Child Development has an administrative team consisting of 3 Program Coordinators. One Division
Administrator and the Division Chief. The Division Staff consists of 25 case coordinator positions. 2-5 developmental specialist or clinical
positions (ARNP), 5 administrative support staff and 2 faculty psychologists. Also within the Division, are 3-6 graduate students used as
OPS staff to provide clinical services.

Position Summary:


The Case Coordinator reports to the Program
Coordinator assigned as their direct supervisor for day to day supervision. Because of the specialization of the program, other members of
the administrative team will direct certain activities and it is expected that the case coordinator will comply with administrative team
direction. Provides targeted case management/care coordination for Florida Early Steps Program eligible children/families. Reviews charts to
facilitate team determination of program eligibility. Completes CMS Early Steps and USF required documents. Performs as a member of a
multi-disciplinary team to develop individualized Family Support Plans. Assists families in meeting their goals via providing family support
and/or community referrals. Regularly travels into community settings and child’s natural environment to perform case coordination.
Documents all work in appropriate venues.

Responsibilities


Responsibilities: Conducts and documents intakes
of children and families entering the Early Steps Program including health history, psychosocial history, demographic information and
screening for development concerns. Identification of established conditions and other information to plan for Multidisciplinary Team
Evaluation and Assessment. Assist family in scheduling for evaluation and assessment.

Develops individualized Family Support Plan in
collaboration with members of a multi-disciplinary team in the Early Steps Program. Provides targeted case management for a caseload of
Early Steps eligible children and families. Completes required CMS Early Steps and USF Forms and bills appropriate payer sources as
required.



Performs and documents ongoing targeted case management including coordination of IF SP periodic reviews at a minimum of
every 6 months and IFSP annual reviews. Minimum quarterly contacts with families to provide support, resource and referral. Coordinates and
assists the family with the delivery of services recommended on the individualized family support plan. Identifies appropriate funding
sources for recommended services on the IFSP. Facilitates community referrals including transition activities prior to age three years when
eligibility ends including coordination of transition conferences with family and Lead Education Agency (LLA)

Other duties as
assigned by administrative team.
